Mailing list submission from a post that doesn't have a form on it

I've received a mailing list submission from a blog post page on my WP site which doesn't currently show any forms. Any ideas how this could have happened?

When I log in to the WP admin, go to Forms, and click on the form which the sign-up came from, I can see the embed URL, but there is no form visible there.
